Coverages:

  1. Conscious Sedation by endorsement with no premium.
  2. Surgical Implant Coverage by endorsement with premium.
  3. Restorative Implant Coverage by endorsement with no premium.
  4. Retroactive Coverage can be applied for.
How can you apply:

  2. Contact the Colorado Dental Association and request an application for CDA membership.
  3. If you are becoming newly licensed in Colorado, then start that process at the same time.
  4. Allow 1-2 weeks for underwriting.
Starting your coverage:

  1. Your coverage can be started when you notify us after underwriting approval.
  2. The State Board will require a policy number to get your license number.
  3. Do Not begin seeing patients until you have coverage started.
